The Clinical Records Assistant plays a critical role in ensuring the accuracy, security, and compliance of all clinical documentation. This position oversees the daily operations of clinical records management, ensuring adherence to federal, state, and organizational regulations while supporting clinical teams with effective systems and processes.

Key Responsibilities

  • Oversee daily clinical records operations and implement policies and procedures
  • Ensure compliance with all applicable state, federal, and regulatory documentation requirements
  • Maintain and safeguard all clinical records—both active and archived
  • Establish and enforce confidentiality and access controls for patient and organizational data
  • Serve as a documentation and compliance resource for internal staff
  • Supervise and guide clinical records personnel and clerical staff
  • Process and release records to authorized parties in accordance with policy
  • Conduct administrative audits post-discharge and coordinate clinical audit reviews

Qualifications

  • Minimum 3 years of clinical records experience (hospice or healthcare preferred)
  • LPN/LVN license preferred, but not required
  • Strong knowledge of healthcare information systems and documentation standards
  • Excellent organizational, communication, and interpersonal skills
  • Ability to work independently with a high degree of accountability
  • Demonstrated flexibility, professionalism, and attention to detail
