
The Chaplain is responsible for all activities of the spiritual services program and the delivery of spiritual services to patients/families. Provide direct spiritual support and/or counsel to patients/families in keeping with their beliefs. Work with staff, clergy, and community groups to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss. Participate in patient care plan meetings by exploring and assessing the potential spiritual needs of patients/families and reporting on services as indicated. Participates in the admissions process by performing informational visits with patients and families and assisting with legal contract signing. Maintain proper records of visits to patients/families as per policy and regulation. Maintain contact with clergy or appropriate representatives of patients/families as indicated. Develop and maintain a resource group of clergy to whom specific aspects of spiritual care may be delegated and arrange on-call availability of chaplain services. Participate in the Support Services on call rotation to meet the needs of Hospice patients 24 hours a day. Perform occasional liturgical assignments such as the annual Service of Remembrance and monthly memorial services with staff. Provide educational programs for community clergy, religious and lay representatives as requested for outreach or educational purposes. Conduct or make arrangements for funeral or memorial services when requested. Provide bereavement follow-up services as defined by established procedures. May be assigned other duties as appropriate.
