The Hospice Business Development Liaison or Care Navigator is responsible for driving patient admissions through strategic relationship-building, referral development, and community outreach. Serving as the face of our hospice organization, this role partners closely with healthcare facilities, physicians, hospitals, and community stakeholders to educate on hospice eligibility, services, and our compassionate approach to end-of-life care.
This is a high-impact, externally focused sales role ideal for a motivated professional who excels at building trust, navigating healthcare systems, and consistently achieving referral growth.
Develop, manage, and expand a network of referral sources to drive consistent hospice admissions
Build and maintain strong relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral partners
Execute strategic sales, marketing, and outreach initiatives aligned with organizational growth goals
Educate referral sources, patients, and families on hospice services, eligibility criteria, and the benefits of early hospice involvement
Serve as a liaison between referral partners and the interdisciplinary hospice team to ensure smooth transitions of care
Address concerns, resolve service issues promptly, and maintain high customer satisfaction
Maintain accurate documentation, including referral tracking, sales activity, call logs, and expense reporting
Represent the organization professionally at community events, in-services, and educational presentations
Adapt to changing market conditions and organizational priorities with professionalism and resilience
Bachelor’s degree in healthcare, marketing, business, or a related field preferred
Minimum of 3 years of successful outside sales experience in healthcare; hospice experience strongly preferred
Demonstrated ability to build, grow, and sustain referral relationships
Strong working knowledge of hospice services, eligibility guidelines, and referral processes
Proven track record of meeting or exceeding sales or admissions goals
Exceptional verbal and written communication skills, including public speaking and presentations
Ability to tailor messaging to diverse audiences, including clinical and non-clinical stakeholders
Strong negotiation and influence skills
Self-motivated, results-driven, and comfortable working independently in the field
Highly organized with strong time management and attention to detail
Entrepreneurial mindset with the ability to problem-solve creatively in a fast-paced environment
Valid driver’s license with reliable, insured transportation
Ability to travel within assigned territory
