Job Description

The Hospice Face-to-Face Nurse Practitioner performs face-to-face encounters for hospice patients, ensuring compliance with Medicare regulations and supporting high-quality end-of-life care. This role involves direct patient interaction, clinical assessments, and collaboration with the interdisciplinary hospice team.

Provides clinical care for hospice patients across all levels of care (General Inpatient, Continuous, Routine and Respite) in all settings of care, including inpatient hospitals, senior living communities and private homes.

Hospice Nurse Practitioner Job Responsibilities:

Conducts face-to-face visits with hospice patients to assess and document continued eligibility for hospice care.

Performs comprehensive physical assessments and evaluates patient status.

Collaborates with the hospice interdisciplinary team to develop and update individualized care plans.

Provides palliative care and symptom management, including prescribing medications as appropriate.

Educates patients and families about disease progression, hospice philosophy, and available support services.

Maintains accurate and timely documentation in compliance with regulatory standards.

Communicates effectively with physicians, nurses, social workers, and other team members.

Ensures compliance with Medicare guidelines for face-to-face encounters.

Participates in undergraduate, graduate medical education, and with other learners as determined by the medical leadership.

Participates in daytime on-call activities.

Minimum Qualifications:

Education: Graduate of an accredited Nurse Practitioner Program; Board Certified Physician Assistant or Acute Care Nurse Practitioner

Certification/Licensure: Current joint registration with the Virginia Board of Nursing and the Board of Medicine as a Licensed Nurse Practitioner (LNP), specializing in Adult, Family or Geriatrics; Inpatient position can include Acute Care certification.

National and State Certification as required for the relevant position (DEA, CDS)

Completion of “healthcare provider” CPR course

Payer Credentialing: Ability to successfully complete the credentialing process by third-party payors and Ability to enrolled as a CMS Medicare/Medicaid payor

Medical Staff Credentialing: Successful completion of credentialing for Medical Staff membership and appropriate clinical privileges sufficient to perform all required services

Hospice Nurse Practitioner Preferred Qualifications

Current Nurse Practitioner license and certification in the state of practice.

Minimum of 2 years of clinical experience, preferably in hospice or palliative care.

Certification in Hospice and Palliative Care through Hospice and Palliative Credentialing Center (HPCC), Advanced Certified Hospice and Palliative Nurse (ACHPN), or equivalent, preferred.

Center for Advancing to Advance Palliative Care (CAPC) Designation, preferred.

Strong understanding of end-of-life care principles and Medicare hospice regulations.

Strong understanding of end-of-life care principles and Medicare hospice regulations.

Excellent communication and interpersonal skills.

Ability to work independently and manage time effectively.

Compassionate approach to patient care and family support.

Able to demonstrate direct involvement in a multidisciplinary team caring for the at-risk patient with advanced age, serious illness, or end of life circumstances.

Skills:

Knowledge of the programs and services in their geographic area

Strong collaboration skills

Demonstrated collaboration/partnerships with other healthcare or community service entities
