Purpose
Our integrated Homeless Youth Program team is seeking a family practice provider to join our Wednesday evening 45th Street Clinic. This clinic serves adolescents (age 12-18) living in shelter and young adults (age 12-26) experiencing homelessness or housing insecurity. The onsite interdisciplinary team includes medical, behavioral health, psychiatry, acupuncture, and a community health navigator. We also collaborate with Neighborcare Health’s daytime clinic services and external community partners, e.g., case management, social work, chemical dependency counselors, and veterinary services.
This is a grant funded position.
