We are seeking a dedicated Case manager with a background in Social Work, Human Services, Psychology, or Behavioral Health to support individuals and families experiencing homelessness. This role is ideal for someone passionate about direct client services, care coordination, crisis interventions, and housing programs.

Position Details:
Employment Type: Full-time, 40 hours/week
Hours: Monday through Friday, 7:30am to 4:30pm
Pay Range: $18.29 to $22.87 per hour, commensurate with education and experience
The Homeless Programs Case Manager provides comprehensive case management and coordinated entry services for individuals and families experiencing homelessness. This includes conducting client assessments, service planning, housing navigation, and referrals to community resources.
This position plays a key role in homeless services, housing stabilization, and Continuum of Care programs, while ensuring compliance with HMIS data entry, grant reporting, and program documentation requirements.
