Job Category: City ManagementGeneral Description:
The purpose of this position is to strengthen Rock Hill’s homeless response system by conducting coordinated street outreach, building partnerships, and providing direct services to individuals experiencing unsheltered homelessness. This position focuses on engaging individuals living outdoors, linking them to housing and supportive services, and supporting system-wide coordination efforts in collaboration with The Catawba Area Coalition for the Homeless (CACH), The Continuum of Care (CoC), law enforcement, and community partners. This job works under general supervision, independently developing work methods and sequences while coordinating closely with partner agencies.
Minimum Education and Experience Requirements:
Requires a bachelor’s degree in human services or related field. Requires three to five years’ experience in a human services field. The equivalent combination of education and related work experience may be considered.
Special Certifications and Licenses:
A valid Driver’s License is required.
Desirable Knowledge, Skills, and Abilities:
- Knowledge of street outreach and homelessness services.
- Knowledge of Rapid Rehousing and case management.
- Knowledge of HMIS, Coordinated Entry, and VI-SPDAT.
- Strong communication and documentation skills.
- Ability to work independently and manage multiple priorities.
- Ability to deal with people beyond giving and receiving instructions. Must be adaptable to performing under stress when confronted with emergency situations or tight deadlines.
- Ability to maintain positive customer-focused relationships with co-workers, supervisors, agencies, the general public, and all other internal and external customers. Personally demonstrates appropriate customer service skills.
- Work is performed onsite. The incumbent must be present at work to perform the essential functions of this job.
Essential Job Functions:
- Serves as a key point of contact between individuals and families experiencing or at risk of homelessness and community service providers.
- Provides Rapid Rehousing (RRH) case management services, including intake, housing stabilization planning, and ongoing follow-up in accordance with program requirements.
- Assists participants with housing navigation, landlord coordination, and connection to supportive services necessary for housing retention.
- Documents services and outcomes accurately in the Homeless Management Information System (HMIS) and other required systems.
- Builds and maintains collaborative relationships with law enforcement, service providers, and community partners to coordinate responses to unsheltered homelessness.
- Serves as a liaison to address homelessness-related concerns and service gaps within the community.
- Participates in coordinated efforts to align outreach activities across partners.
- Conducts Coordinated Entry assessments, including vulnerability assessments, to support prioritization for housing and services.
- Completes timely and accurate HMIS data entry to support system-wide reporting and analysis.
- Promotes data quality and participation among partner agencies as needed.
- Supports Point-in-Time (PIT) Count activities, outreach planning efforts, and related data collection initiatives.
- Assists with community presentations and public awareness activities related to homelessness.
- Maintains contact lists, prepares reports, and completes required documentation related to outreach and case management activities.
- Tracks activities and outcomes to support program monitoring and improvement.
- Performs other duties as assigned to support the City’s homeless response system.
- Performs all other duties as may be assigned to meet organizational needs.
