Per Diem RN Case Manager – Homecare

Are you a school nurse or RN looking for a flexible summer opportunity that allows you to continue making a meaningful impact in patient care?

The VNA of Care New England is seeking compassionate and motivated Per Diem RN Case Managers to join our Homecare team. This role offers the flexibility to work around your summer schedule while providing one-on-one patient-centered care within the community.

As an RN Home Health Clinician, you’ll manage a caseload of patients from admission through discharge while collaborating with a dedicated interdisciplinary team to support positive patient outcomes.

What You’ll Do

  • Perform comprehensive patient assessments and develop individualized care plans
  • Coordinate care and referrals across disciplines and specialties
  • Monitor patient progress and adjust care plans as needed
  • Educate patients and caregivers on managing care at home
  • Communicate effectively with patients, families, and healthcare providers
  • Maintain accurate clinical documentation
  • Support continuity of care and quality patient outcomes

Qualifications

  • Current Rhode Island Registered Nurse (RN) license required
  • Bachelor’s Degree in Nursing or related field strongly preferred
  • Minimum of 1 year clinical nursing experience required
    • Home health or acute care experience preferred
  • Current CPR certification
  • Valid driver’s license and automobile liability insurance
  • Strong communication, organization, and clinical assessment skills
