Job Description

Schedule: PRN, minimum requirement of 2 shifts in a 4 week period (Monday-Sunday, anytime)

This position provides personal care and home management tasks at patient’s residence, under the supervision of a RN Case Manager.

What are the essential duties day to day?

  • Follows the nurse aide care plan to provide compassionate care to the patient in the areas of personal hygiene, elimination, feeding, and mobility.
  • Reports changes in patient’s condition, concerns about patient and/or family to appropriate nursing staff in a timely fashion and participates in interdisciplinary team conferences as necessary.
  • Works collaboratively with the IDG and other departments in achieving patent care goals.
  • Documents patient care according to agency policy and procedures; documentation is clear, complete, concise, and timely.
  • Responsible for professionally managing time, people, medical supplies, and technology.
  • Establishes and adheres to a timeframe for patient visits in coordination with patients, families and facility staff members.
  • Maintains a clean, safe and comfortable environment for the patient.
  • Teaches and/or reinforces home safety, energy conservation and sanitation.
  • Provides light housekeeping tasks.
  • Assists with, or provides, meal preparation and related tasks.
  • Demonstrates excellent communication skills and creates successful interpersonal relationships with patients and families.
  • Complies with organizational nurse aide standards.
  • Participates in staff training and education programs.
  • Average of 4-6 visits a day
