Provides home visitation services, through the Help Me Grow Home Visiting Program in Allen County, to expectant moms and families with young children that cultivate and strengthen nurturing parent-child relationships, promote healthy childhood growth and development, and enhance family functioning by reducing risk and building protective factors.

FUNCTIONS:

  • Provides culturally sensitive home visitation services for families that promote positive parent-child interaction, child developmental skills, and healthy and safety practices through the use of curriculum and other educational materials in accordance with best practices model standards. Provides information, referrals, and linkages to available community resources. Links families to a medical provider to assure optimal health and development.

  • Administers developmental assessment tools and screenings to monitor progress of child health and development and parent-child interaction skills in accordance with timelines

  • Works with families to develop individualized family service plans and to meet stated goals.

  • Maintains accurate and complete electronic, case files, and program records in accordance with timelines.

  • Facilitates relationships and coordinates service with program collaboration partners and other community agencies.

  • Participates in supervision on a regular basis and participates in required trainings.

  • Performs all other duties as assigned

Education:

Associate's Degree in Social Work, Early Childhood Development, Teaching, or Nursing (preferred)

LICENSURE/CERTIFICATIONS

Candidates must become a credentialed home visitor through the Ohio Department of Health during the first three months of hire.

MINIMUM QUALIFICATIONS:

  • Preferably two years of recent experience working in the community experience with an emphasis on home visiting.

  • Strong understanding of child development and community resources.

  • Ability to write effectively and speak clearly. Reliable transportation, valid driver’s license, and proof of insurance.

  • Ability to work well with and relate effectively with co-workers, staff, or other service providers, parents, young children, and community groups.
