Hourly Pay Range:

$36.00 - $55.80 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

• Position: Registered Nurse Home Visits – Family Connects - Days
• Location: Chicago, IL
• Full Time
• Hours: 32 hours per week; Monday-Thursday 8:00 am – 4:30 pm
• Required Travel: Home Visiting Nurse provides home visiting to all Swedish Hospital newborns and their families within the City of Chicago.

General Summary:
• The Family Connects model was developed within the context of the Durham Family Initiative, funded by the Duke Endowment in 2002, with the primary goal of reducing community rates of child maltreatment (K. Dodge, Principle Investigator). The program centers on a theory-driven and evidence-based assessment of child and family strengths and needs, provided in an informal and family-friendly manner. As a Family Connect Community Home Visiting nurse you will providing home visiting services to all families and newborns born at Swedish Hospital and reside within the city of Chicago. The goal is to support and assess family needs and their physical and psychological well-being and facilitate referrals and connections to community resource. Family Connects supports new parents in caring for their newborn(s), offers physical assessments of the mother and the baby, addresses questions about caring for the newborn(s), identifies parents’ needs, and helps to identify community services, education, or resources that can meet those needs. In addition, through this connection to resources, Family Connects helps to identify gaps in critical community-wide resources with the goal of working toward increasing needed services locally. You will also be promoting a safe and caring environment to welcome their new babies in accordance with the mission, vision and philosophy of Swedish Hospital.

What you will need:
• B.S. degree in nursing with current RN license, or B.A. degree in another field with current RN license, or A.S. degree in nursing with state-level RN, and at least one year of relevant nursing experience.
• American Heart Association CPR certification.
• Must have a valid driver’s license and an automobile for use in making home visits.
• Registered nurse experienced at a hospital or clinic OB preferred.
• Computer skills required, including Word, Outlook and Excel and other various computer programs. Proficient and comfortable in using laptops as part of in-home assessment and intervention.
• Bilingual Spanish and English preferred. Ability to utilize translation services with clients.
• Previous experience working in a clinical setting and extensive experience in dealing with customers preferred.
• Must have excellent organizational skills and the ability to prioritize tasks, independently with minimal supervision.
• Ability to work with culturally diverse populations.
• Preferred skills include one or more years of experience in child/maternal health, social work, or other related field, and or Certified Lactation Educator or Consultant.

What you will do:
• Visit families during the immediate postpartum period in the comfort of the patient’s home environment or virtual appointment based on patient preference
• Follow the Family Connects home-visiting protocol. Keep records of client observations and activities as required. Document information for family assessments and program evaluation. Record information in real time via in home use of computerized record system.
• Visit inpatient settings as needed to schedule home visits for new mothers.
• Assess the family needs in 12 domains of physical and psychological well-being including post-partum depression screening.
• Assist in referrals in connection to community agencies to support families.
• Facilitate and teaching various community-based classes in our community center.
• Apply effective time-management skills, organization and multitasking. Documentation is expected to be done and communication with providers within 48 hours of home visit.
• Maintain 10 open visits slots and conduct and document at least 7 home visits per week.
• Respond to family’s needs through flexible scheduling including home visits during the evenings or weekends.
• Work cooperatively as an active member of the Family Connects team by attending weekly team meetings, clinical supervision, staff meetings, and other meetings as required. Support and encourage development of other staff and of the Family Connects Program.
