The Utah Department of Health and Human Services Home Visiting Program was
established in 2008 through a grant from the federal Department of Health and Human
Services, Administration for Children and Families. To support evidence-based home
visiting models in Utah, the Home Visiting Program promotes a coordinated service
continuum of evidence-based home visiting that empower and create positive change in
the lives of families at risk. This is not a client facing role
Utah, the Home Visiting Program supports high-quality service delivery by using data,
continuous quality improvement, and state and federal compliance standards to
strengthen home visiting services and improve outcomes for children and families.
Monitoring and Quality Assurance Team, providing oversight, monitoring, and support to
home visiting sites across the State of Utah.
