Pay Grade
The Home Health Tech assists in personal care and certain procedures of rehab therapy to clients in their place of residence and in the provision of home health care services that reflect the home health care agency philosophy and standards of home health and nursing care of assigned clients. The Home Health Tech ensures quality and safe delivery of home health care services and assists office staff and management with clerical duties as assigned.
Responsibilities and Duties
Implements an individualized patient care plan under the direction of the RN and/or PT/OT if the RN services are not involved in the patient's episode of care. Consistently prioritizes and delivers safe effective patient care.
Follows guidelines for Tech competence by maintaining and updating their knowledge and skills.
Interacts with and gives feedback to the RN or PT/OT related to the patient status and/or when safety issues are identified in the home.
Meets annual regulatory requirements for education.
Considers factors related to cost and impact on visits and care in their delivery of services.
Prioritizes visits in logical manner considering geographical area and patient preferences to maximize efficiency.
Promotes and strives to protect the health, safety and rights of the patient by providing safe patient care that adheres to TJC NPSG.
Promotes and strives to achieve employee satisfaction and engagement with the mission of the agency and VHS as a whole.
Recognizes and reports changes in skin conditions such as a rash, pressure ulcers, etc.
Utilizes appropriate and safe techniques in performing personal hygiene and grooming tasks that include bed bath; sponge, tub and shower bath; hair shampooing in sink, tub, and bed; nail and skin care; oral hygiene; and toileting and elimination.
Assists with safe transfer techniques and ambulation.
Assists with normal range of motion and positioning.
Assists in assuring adequate nutrition and fluid intake.
Education
High School Diploma or GED preferred.
Successful completion of 40-hour educational training program with written post-test and technical competencies to include didactic education required.
Experience
One year of current clinical experience required.
Certification & Licensures
West Virginia and/or Virginia state Certified Nursing Assistant certification required, based on state of work assignment.
BLS Certification (Basic Life Support) - American Heart 'Healthcare Provider' (HCP) - AHA approved required. New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.
Driver's license and reliable transportation required.
Qualifications
Understands basic principles of human growth and development.
Ability to read, write and carry out directions.
Must be able to perform clerical tasks as assigned.
Must be able to complete skills demonstrations for competency as required by Medicare.
