We are seeking a compassionate, organized, and detail-oriented Scheduler to join our Home Health team.

What You’ll Do

• Coordinate and schedule patient visits for clinical staff
• Manage clinician schedules to ensure timely and efficient patient care coverage
• Support key workflows throughout the patient care journey
• Communicate with Clinical Team Managers, field clinicians, physicians, patients, and facility staff
• Adjust schedules and staffing plans based on changing patient and operational needs
• Monitor scheduling productivity and support branch operational goals
• Respond quickly to scheduling conflicts and unexpected changes
• Maintain accurate scheduling documentation and communication
• Support patient satisfaction through proactive communication and coordination

What You’ll Need

• High school diploma or GED required, Associate degree preferred
• Minimum of 1 year of practical work experience
• Previous home health scheduling experience preferred
• Strong communication, organization, and problem-solving skills
• Ability to manage multiple priorities in a fast-paced environment
• Ability to make decisions and adapt quickly with changing information
• Intermediate Microsoft Office skills
• Ability to learn systems such as HomeCare HomeBase and Workday
• Understanding of patient care coordination and scheduling workflows preferred
• Bilingual Spanish/English preferred

This position reports directly to the Administrator or Branch Director and plays an essential role in supporting both patients and clinicians throughout the care process.
