Full-time schedule, Monday through Friday 8a-5p

Provide hands-on, personalized care in patients’ homes
Manage and coordinate a caseload from start to finish
Perform assessments and develop individualized care plans
Monitor patient progress and adjust care as needed
Collaborate with physicians and interdisciplinary teams
Educate patients and families for long-term success
Ensure compliance with all clinical and regulatory standards (including HIPAA)
Active, unrestricted RN license
1+ year of clinical nursing experience
Ability to travel within the coverage area (up to 50%)
Reliable transportation, valid driver’s license & insurance
Strong clinical judgment and communication skills
Ability to lift, move, and assist patients (50–100 lbs intermittently)
Preferred:
Home Health or Hospice experience
Experience with HomeCare HomeBase
ADN or BSN
