Home Health Physical Therapist (PT)

We’re hiring a Home Health Physical Therapist (PT) who wants competitive earning potential, independence in the field, and the opportunity to help grow an expanding home health territory.

You’ll step into a home health role focused on evaluations, treatment, rehabilitation, and helping patients safely regain mobility, function, and independence in their own homes. You’ll have significant autonomy over your day while remaining connected to an experienced clinical team.

What makes you a great fit

• You are a licensed Physical Therapist in Connecticut or eligible for licensure
• You have professional Physical Therapy experience
• Home health experience is strongly preferred
• You’re comfortable independently managing a mobile patient caseload
• You enjoy helping patients regain mobility, function, and independence at home
• You value autonomy and a smaller, high-touch work environment

Compensation & Benefits

• Base salary: $105,000–$130,000
• Additional productivity incentives available, with final structure being established

• PTO: 15 days

• Mileage reimbursement: $0.725 per mile between patient homes
