Private Duty Nurse (RN/LPN) – 1:1 Home Care

Location: Clio/Flint/Saginaw/Burton/Lapeer
Setting: In-home, one-on-one care

Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.

Specialized Infant & Pediatric Care

• Location: Flint, MI

  Schedule: Sun, Mon, Wed, Thu, Fri, Sat | 8:00 AM – 4:00 PM

  We are seeking a compassionate and skilled nurse to provide 1-on-1 care for a delightful 6-month-old baby boy in a vibrant, loving home. Our ideal candidate is someone who balances clinical excellence with a warm heart—this little guy thrives on cuddles and interaction!

• Location: Saginaw, MI

  Patient: 1-Year-Old Female

  Schedule: 24/7 Coverage Needed (12-Hour Shifts)

  • Day Shift: 7:00 AM – 7:00 PM (Sun–Sat)

  • Night Shift: 7:00 PM – 7:00 AM (Sun–Sat)

Complex Adult Care

• Location: Lake Orion, MI

  Patient: 49-Year-Old Female (Paraplegic)

  Schedule: 24/7 Skilled Nursing Coverage (12-Hour Shifts)

  • Day Shift: 7:00 AM – 7:00 PM (Everyday)

  • Night Shift: 7:00 PM – 7:00 AM (Everyday)

  We are seeking a highly skilled and organized RN or LPN to join a comprehensive multi-disciplinary team for a 49-year-old female patient. This role involves managing a complex clinical profile within a well-equipped home environment. You will work alongside a 12-hour/day aide, PT/OT, and specialized therapists to provide the highest standard of recovery-focused care.

• Clio (56-Year-Old Male): Quadriplegic care including wound care and bowel program. Requires a dedicated caregiver to assist with appointments.

  • Shifts: 6 AM – 4 PM (Tue & Thu) (Male Nurse per patient request)

Responsibilities

• Provide skilled nursing care (trach/vent care, medication administration, catheter care)
• Monitor and document patient condition and vital signs
• Assist with daily living activities (ADLs)
• Communicate with families and care teams
• Maintain safe, high-quality, patient-centered care
  
  

Schedule & Pay

• Full-time, part-time, and per diem shifts available
• Day, night, and weekend shifts
• Weekly pay + optional daily pay

Qualifications

• Active RN or LPN license in the state of Michigan
• Home care or private duty experience preferred (not required)
• Trach/vent experience a plus (training provided)
• Strong communication and critical thinking skills
