  Conduct comprehensive assessments of patients’

  environment evaluations.

  Develop and implement individualized care plans in

  Administer medications, wound care, and other
  treatments as prescribed, ensuring adherence to safety and infection control
  protocols.
• 
  Monitor patients’ progress and responses to
  treatment, making adjustments to care plans as necessary and communicating
  findings to healthcare providers.
• 
  Educate patients and their families about
  managing medical conditions, medications, and necessary lifestyle changes,
  providing resources and support.
• 
  Maintain accurate and up-to-date patient
  records, documenting assessments, interventions, and patient outcomes.
• 
  Coordinate care with other healthcare
  professionals, including physical therapists, social workers, and physicians,
  to ensure comprehensive support.

  Stay informed about best practices in home
  healthcare and changes in regulations to provide high-quality care.

Requirements

• 
  Bachelor’s degree in Nursing (BSN) preferred; an
  Associate’s degree in Nursing (ADN) may be accepted with relevant experience.
•
