Duties of Position

The Medical Social Worker provides medical social services to clients in their homes in accord ance with physician orders and under the direction and sup ervision of the Director of Nursing or other app ropriate supervi sor.

Responsibilities of Position

Assesses the client's potential to cope with his/her social and health p roble m Acts as con sultant to the members of the health team; helps the team understand the social, e motional and envi ron men tal factors related to the client 's health p roble m Helps clients to utilize the resources of their families and the community. Provides rehabilitative and supportive ca s Assists clients and their families cope with personal and environmental difficulties which might predi spose them t o illness or might interfere with their ability to obtain maximum benefits. Assists in the client's admission process to the Agency by performing the initial evaluation and assessment of the clien t's psy cho social status as well as an assessment of the client's home and family to identify socioeconomic and emotional factors that will affect the treatment plan. Assists in d evelop ment and i mple mentation of the interdisciplinary client care plan as it pertains to medical s ocial work. Observes, records and reports changes in t he client 's e motional and social factors that affect the client 's illness and his/her need for care as well as his/her response to treatment. Con sults with the attending physician c oncerning alteration of the plan of t reat m Enters clinical notes into EMR within required hours of scheduled visit (24 hrs. for non-Oasis visits / 48 hrs. for Oasis visits) and progress notes and other clinical record forms outlining the services rendered. Evaluates the client's and fa mily 's responses to, and eff ectiveness of, the medical social work intervention. Confir ms, on a weekly basis, the scheduling of visits with the D irector of Nursing to facilitate coordination of other staff visits. Participates in Agency activities and co mmittees that include staff development and in-service education when appropriate. Supe rvises the social work assistant once mon t Participates in discharge pl an Assumes responsibility for s elf-development by continually striving to imp rove his/her Medical Social Worker practice through formal education, attendance at workshops and conferences, active particip ation in p rofessional and relat ed org anizational meetings, and individual research and reading. Complies with Agency dress code policy.

Maintains efficient flow of Agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time off requests.

Maintains current TB testing (refer to Health Screening Policy).

Job Conditions

Position requires the ability to communicate well orally and in writing. On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds and to lift and/or car ry up to 30 pounds. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client's condition; to perform and demonstrate client care; and, to assess a client's home that may not be wheelchair accessible.

Qualifications:

  • Masters Degree from a School of Social W ork app rov ed by the Council of Social Work Education.
  • Two years' experie nce with at least one ye ar of experi ence in a h ealth care setting (ho spital, clinic, rehabilitation center, etc.)

Compensation Range:

$18.00

to

$27.00

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
