Responsibilities of a Home Health LPN

• Administer skilled nursing care in accordance with the physician's instructions and patient's care plan
• Monitor patient's condition and assess home environment
• Engage with patients' families, providing appropriate education and information regarding care
• Document clinical notes and maintain accurate health records
• Collaborate with interdisciplinary team to coordinate personalized care strategies

Key Qualifications for the Home Health LPN Role

• Valid LPN license in the state of PA
• Experience in EMR, Other EMR, TruChart EMR
• Certifications in any of AHA ACLS or AHA BLS
• Previous home health or clinical experience is favorable
• Strong communication and interpersonal skills
• Ability to work independently and manage time effectively
