The Home Health Licensed Practical Nurse provides direct patient care in the home setting as a key member of an interdisciplinary team that includes Registered Nurses, Physical Therapists, Occupational Therapists, Speech‑Language Pathologists, Social Workers, and Home Health Technicians. The Licensed Practical Nurse conducts patient assessments and implements the individualized plan of care established by the Registered Nurse to ensure that each patient’s clinical needs are met.

This role delivers one‑on‑one patient care and communicates any changes in patient status to the RN Case Manager in a timely and accurate manner. The home health environment is technology‑driven; therefore, the Licensed Practical Nurse uses email as a primary communication tool and completes all documentation electronically using a laptop‑based medical record system.

The patient population represents a broad range of ages, diagnoses, and levels of acuity. The Licensed Practical Nurse practices under the direction of a Clinical Team Manager. Employment begins with a comprehensive 6–10-week orientation alongside an experienced preceptor to ensure clinical readiness and confidence before independent practice.

Additional responsibilities may include participation in performance improvement initiatives, policy and procedure review and development, and engagement in research activities. Home health practice offers diverse clinical experiences, dynamic work environments, and the unique opportunity to support patients within their daily living spaces—making a meaningful impact on their health and quality of life.

Education

Completion of an accredited school of nursing program is required.

Experience

Must meet all requirements of an LPN I ladder to advance to LPN II.

Certification & Licensure

Licensed Practical Nurse licensure required. Based on primary state of residency and in accordance with current Virginia Board of Nursing Regulations, must be licensed or eligible to practice pending licensure as a Licensed Practical Nurse in the Commonwealth of Virginia and/or West Virginia (as applicable depending on geographical area) with either a: Multi-state license, under the Nurse Licensure Compact.

BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved is required. New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.

Valid driver’s licensure and reliable transportation is required.

Relevant specialty certification is preferred.
