We are seeking a compassionate and knowledgeable professional to join our team as a Home Health Clinical Support RN. This role is critical in offering exemplary patient care at home, ensuring patients receive thorough and attentive medical support.

Core Responsibilities

• Provide clinical support and guidance to patients and families in a home health setting
• Perform comprehensive patient assessments and create personalized care plans
• Collaborate with a multidisciplinary healthcare team to ensure comprehensive care
• Monitor, evaluate, and document patient progress, adjusting care plans as necessary
• Educate patients and caregivers on health management and medical protocols
• Ensure adherence to health regulations and organizational policies

Work Schedule

● Monday through Friday, day shifts; no required weekends, on-call, or holidays

Coverage area: Glenview, Evanston, Skokie, Niles, Chicago, Park Ridge & surrounding

Required Qualifications

• Current RN license in state of practice
• Minimum of 2 years of clinical experience, preferably in home health
• Experience with Electronic Medical Records (EMR) systems, including TruChart EMR
• Strong communication and interpersonal skills
• Ability to work independently and make informed clinical decisions
• Exceptional organizational skills and attention to detail
• Proficient in medical documentation and record-keeping
