Your Territory: Southwest Minneapolis & Inner Metro You'll manage patients within a concentrated area of the southwest Minneapolis and inner-ring metro , including: Edina, St. Louis Park, Richfield, Hopkins, and Southwest Minneapolis , including areas within the 55410 and 55416 ZIP codes . Patient scheduling and territory assignments are designed around geographic needs, with visits generally concentrated within approximately a 35-mile radius of your home whenever possible . What You'll Own As an RN Case Manager, you'll oversee an assigned home health caseload and coordinate care throughout each patient's episode. You'll: Manage an assigned home health patient caseload Complete OASIS Start of Care, Recertification, and Discharge assessments Perform comprehensive nursing assessments Develop and update individualized plans of care Provide skilled nursing visits Monitor patient progress throughout the episode of care Perform wound care, medication management , and other skilled nursing services Educate patients and caregivers on diagnoses, medications, treatments, and recovery Identify and communicate changes in patient condition Coordinate directly with physicians and healthcare providers Collaborate with PT, OT, SLP, social work, and interdisciplinary team members Help patients safely transition home following hospitalization or rehabilitation Work proactively to prevent avoidable emergency department visits and rehospitalizations Complete timely documentation using Homecare Homebase (HCHB) You're Still a Nurse. We're looking for: An active Minnesota Registered Nurse (RN) license Three or more years of RN experience Home health experience preferred OASIS experience highly valuable Strong comprehensive assessment and critical-thinking skills Excellent organization and time management Confidence working independently in patients' homes Strong patient, caregiver, physician, and interdisciplinary communication Reliable transportation for visits throughout your assigned territory Experienced nurses from hospital, acute care, rehabilitation, skilled nursing, and other clinical environments are encouraged to connect with us.
