Home Health Aide – Hospice – Days- Hudson Valley

Lawrence Home Care of Westchester (LHCW) is a Medicare certified Home Care Agency providing skilled and supportive services to homebound patients. As part of Lawrence Community Health Services, we provide home health aide (HHA) support to our clients in Southern Westchester and the Northern Hudson Valley area. We provide these services in patient’s homes, including assisted living facilities. Cross coverage may occur for the Jansen Hospice patients in the Northern Bronx and Southern Westchester area. These patients have end of life supportive needs.

Lawrence Home Care of Westchester (LHCW) has an interdisciplinary team of a Medical Director, nurses, social workers, rehab occupational and physical therapists, and a speech language pathologist who works together caring for families and patients. Certified HHA’s provide care and assistance with patients’ personal needs as well as offering caring and support to family members.

This is a full-time day shift opportunity from 8:30am – 4:00pm, Monday – Friday. This position is at NYP/Lawrence Community Health Services of Westchester, a certified home health agency.

Preferred Criteria

  • One year of Hospice or Home Care experience

Required Criteria

  • Completion of a basic training program in home health aide services approved by NYS DOH, or an equivalent exam approved by the Home Care Department. (must submit written proof of completion of such a certification/exam)
  • Certification as a Home Health Aide (HHA)
  • Basic understanding/commitment to the concept of Hospice Care for terminally ill patients
  • Must have valid driver’s license and car to use for work.
