Primary Work Location:

HK1/Oakdays is in the Elmhurst neighborhood of Oakland, near the Oakland Airport and Coliseum. Cardea Health provides comprehensive clinical services in a Permanent Supportive Housing program located in Alameda County known as Project Homekey. The Homekey program houses a population that has experienced complex medical illness and disability as a result of homelessness. Project Homekey is designed to adapt clinical services to each individual up to and including end of life care. The Homekey model is a unique service model developed to provide supported housing to this exceptionally vulnerable population.

The HHA provides essential care and support to patients as part of a comprehensive care team including Registered Nurses, Licensed Vocational Nurses, and community providers. Responsibilities include assisting patients with activities of daily living such as bathing, dressing, and grooming, monitoring vital signs, feeding patients, and helping with mobility. HHA’s also provide emotional support to patients and report any changes in patient condition to the nursing staff. HHAs play a crucial role in ensuring patients' comfort, safety, and well-being at HK1/Oakdays.

Qualifications

Responsibilities:

The HHA will assist clinical staff and HK1/Oakdays residents in the coordination of medical and behavioral health care. Duties include:

  • Conduct patient/personal care in accordance with resident plan of care
  • Support resident relationship with primary care and other professional disciplines
  • Assist nursing team in communication with residents including arranging and facilitating appointments with nursing staff
  • Ensure adherence to infection control standards on site among staff and residents
  • In collaboration with the housing agency, ensure daily wellness of residents. Participate in immunization programs at HK1/Oakdays
  • Assist in the execution of population health and safety interventions on site
  • Answer calls to nursing station and respond independently to when appropriate
  • Assist nursing staff with light clerical duties including filing and making copies
  • Ensure that clinical documentation is current, timely, and accurate
  • Maintain confidentiality in accordance with the Health Insurance Portability and Accountability Act (HIPAA)
  • Recognize and respond to medical emergencies on site, follow up with MD communication or 911 activation
  • Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect
  • Other duties and responsibilities as required

Minimum Required Skills &

Qualifications:

  • High School Diploma or equivalent
  • Must be authorized to live and work in the U.S
  • Must be 18 years of age or older
  • Must have personal, reliable transportation to and from work sites and shifts

Preferred

Qualifications:

  • Experience with homeless or incarcerated populations is highly desirable
  • Two years of experience in patient care
  • Self-directed and able to work with minimal supervision
  • Detailed oriented and highly organized
  • Compliance with all company policies
  • Must provide proof of COVID vaccination or willing to be vaccinated
  • Prior experience with homeless or incarcerated populations is highly desirable
  • Proficiency in Microsoft Outlook, Word and Excel is required

Physical Requirements:

  • Frequent sitting or stationary work
  • Frequently lift and/or move up to 20 pounds
  • Occasionally lift and/or move up to 50 pounds

Work Environment:

  • Employees have direct contact with clients/patients while administering care
  • This position will require traveling to other office locations or off-site meetings as required by the job
  • Employees may be occasionally exposed to toxic or caustic chemical
