Helio Health, Inc. is on a mission to promote recovery from the effects of substance use and mental health disorders and other health issues. To support the mission of Helio Health, Inc the Home Health Aide for the Aging In Place Program assists individuals with activities of daily living and health related tasks to help the individuals maintain independence and function at the highest level.

Schedule: Tuesday-Saturday or Sunday-Thursday, Days

Pay rate: $22.00/hr.

In addition to Helio Health’s comprehensive benefits package, this position is eligible for a Sign-On incentive up to $1,500!

Responsibilities

• Takes direction from the Nursing Team Leader, Team Leader, Clinic/Program Director and/or Service Director and keeps them informed regarding content and volume of workload.
• Works with individuals/patients in Residential programs and homes, and internal and external community agencies and organizations.
• Provide quality and safe delivery of home health aide services that meet the needs of the client as determined by professional assessment.
• Assists individuals with daily living and health related tasks.
• Assist with meals by serving and feeding participants as needed and cleaning up after meal completion.
• Monitor residents' health by noting any changes in physical and/or emotional health and communicate any changes to staff and Supervisor.
• Assist individuals, as necessary, with medication administration, preparation of complex modified diets, prescribed exercise programs, use of prescribed medical equipment, supplies and devices, special skin care, dressing changes and/or other daily activities
• Other duties as assigned

Qualifications

• High School Graduate/Equivalency required
• Active Certified Home Health certification required.
• One year in health or Human Service experience preferred.
• Valid NYS Driver's License
