Job Title: Home Care Supervisor

Location: New Jersey PACE

Hours-Monday - Friday 8-4:30


We are seeking a dedicated Home Care Supervisor to oversee Certified Home Health Aides (CHHAs) and Certified Nursing Assistants (CNAs) delivering care in home settings. This role partners closely with the Director of Nursing (DoN) to ensure accurate scheduling, adequate staffing, and high-quality participant care. The Home Care Supervisor actively participates in interdisciplinary team (IDT) meetings and ensures staff are trained, credentialed, and compliant with Trinity Health PACE standards.

Key Responsibilities

  • Supervise CHHAs/CNAs to ensure safe, high-quality care aligned with care plans
  • Assist with scheduling, staffing coordination, and timekeeping accuracy
  • Participate in IDT meetings and contribute to care planning and assessments
  • Support recruitment, onboarding, training, and competency validation of staff
  • Monitor compliance with policies, regulations, infection control, and safety standards
  • Conduct staff supervision, audits, and performance oversight
  • Maintain accurate documentation in the electronic medical record
  • Coordinate home care services, supplies, and participant needs
  • Address concerns, resolve service issues, and escalate when necessary
  • Ensure staff certifications (e.g., CPR) remain current
  • Provide on-call support for staffing and operational needs

Qualifications

  • High school diploma or equivalent required
  • CNA or Home Health Aide certification required
  • 2–4 years of related experience or equivalent combination of education and experience
  • Prior supervisory experience required
