Job Description

The Smithton District Hospital (SDH) and Community Health Centre are small sized sub-acute public hospital located in Smithton Tasmania. It services the whole Circular Head region with a catchment population of 8000 people.

Smithton Home Care services provide a 5 day a week service, providing home and personal care to clients in their own home plus a foot care clinic service.

The Team:

Smithton Community Services is a small team of health professional which operate out of the Smithton District Hospital to provide care to clients either in their own home or through Community Nursing or Carer clinics at the hospital.

The Role:

  1. To coordinate Home Care services and provide admin support for community services
  2. Working within the Community Home Support Program (CHSP) Aged Care Quality Standards, supervise, coordinate and allocate care work to Home Help and Personal Carer staff providing services to the frail, aged and disabled within the community.
  3. Receive and respond to all referrals, ensuring confidential handling of enquiries and routine matters as required, or directing clients to other agencies when appropriate.
  4. Undertake home care/domestic assistance assessments and reassessments in a community setting for the frail aged, people with disabilities and their carers.
  5. Monitor and review services for clients through discussion with field staff, attendance at case conferences and through ongoing client contact ensuring an efficient flow of information to and from the Community Nursing and Home Care Service.
  6. Collaborates regularly with other service providers regarding the provision care for clients.
  7. Prepare written reports, provide statistics and maintain client health care records and accurately recording all necessary information into a computerised system as per organisational procedures.

Details of appointment:

  • Permanent, full time, day worker position, working 76 hours per fortnight, commencing from as soon as you can join us. *notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $73,811 to $79,779 per annum. Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Public Sector Unions Wages Agreement 2022

Eligibility:
