Executive Home Care of Omaha is looking for a Referral Marketing Representative to build strong referral relationships with Skilled Nursing Facilities (SNFs), rehabilitation centers, Assisted Living Facilities (ALFs), and Memory Care communities throughout the Omaha area.
This is a relationship-driven business development role—not a clinical position. You’ll serve as a trusted representative of EHC, connecting with discharge planners, social workers, case managers, and facility teams to make EHC their go-to resource for safe, dependable in-home care.
If you have experience as a social worker, discharge planner, or case manager, you already understand how referral decisions are made and what families and facility teams need from a trusted home care partner. Now, you can use that experience to build relationships, create opportunities, and help grow something meaningful.
What You'll Do
- Build and maintain strong relationships with discharge planners, social workers, case managers, and facility staff across the Omaha healthcare community
- Conduct regular in-person visits and outreach to SNFs, rehab centers, ALFs, and Memory Care communities
- Introduce facility partners to EHC’s services, including our Sensi Co-Pilot and GUIDE Program
- Serve as a knowledgeable, responsive point of contact for referral partners
- Follow up consistently with facilities and referral sources to strengthen relationships and generate qualified referrals
- Track outreach activity and referral opportunities and communicate progress to leadership
- Identify new partnership opportunities and help expand EHC’s referral network
- Work directly with Steve and Jerri on outreach strategy, messaging, and territory development
- Represent EHC at community events, lunch-and-learns, in-services, and other professional gatherings as needed
What We're Looking For
The ideal candidate understands healthcare referrals from the inside.
- 5+ years of experience in social work, discharge planning, case management, or a related healthcare setting
- Strong understanding of the senior care and post-acute healthcare landscape
- Existing relationships or familiarity with Omaha-area SNFs, rehab centers, ALFs, and Memory Care communities is a major plus
- Confident, professional, and comfortable with face-to-face relationship building
- Excellent communication and follow-through
- Self-motivated and able to manage your own schedule, territory, and outreach activities
- Reliable transportation and willingness to travel throughout the Omaha metro area
- Genuine passion for helping seniors and families
- A relationship-first mindset and alignment with EHC’s mission
Schedule & Compensation

Part-time to start with a flexible Monday–Friday schedule
- Schedule designed around facility visit and outreach hours
