Our Vision is to be the trusted partner in quality outcome-based home care! The AZ HomeCare Manager (CM) is responsible for performing initial client intakes, client reviews, client maintenance, and staffing. Often the CM will handle client issues relating to care, and staffing, as well as being the liaison between MCO Case Managers (insurance provider) and Office if necessary. The CM will assist in matching caregivers to clients, and this position requires daily driving. The CM will be reimbursed for mileage. Immediate cell phone stipend.

ROLE AND RESPONSIBILITIES

  • Must have reliable transportation as traveling valley wide is required
  • Initial Client Intake visits (in the client’s home)
  • Conduct client-in-home visit reviews (5-days, 30-days, and 90-days)
  • Create Care Plans for all clients
  • Document any issues and/or positive information in the Client Logging and/or Caregiver Logging system in CL.
  • Updates information as needed in a timely manner within our EMR system
  • Update Excel Workbook trackers – Client related
  • Report any concerns to the MCO CM and the Office immediately via email
  • Any other duties as deemed necessary by the Director
  • May have to provide care for clients at any given time due to call off’s if not able to restaff with a caregiver
  • Will be compensated weekly for “on-call – after-hours” emergency calls

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • High school graduate or GED preferred
  • Excellent computer skills and must be detailed oriented/accurate
  • Must have good interpersonal communication, time management, and problem-solving skills.
  • Must pass a background check as well as the ability to obtain Finger Print Clearance Card

PREFERRED SKILLS AND/OR EXPERIENCE

  • ?Bilingual is a plus (English and Spanish)

Outreach Assured Care offers full benefits package offers such as: health insurance, 401k, 9-paid holidays, and PTO accruals after the probationary period.

Pay Range: $21/Hr
