The Home Care Intake Specialist supports the home care admission process by partnering with Home Care Transition RNs to coordinate referrals, enter referral information into the electronic medical record (EMR), and ensure patients are scheduled for timely Start of Care. This role serves as a key point of contact for hospitals, physician offices, and other referral sources while providing exceptional customer service and ensuring accurate, efficient referral processing.

Key Responsibilities

  • Process and enter home care referrals into the EMR accurately and timely.
  • Partner with Home Care Transition RNs to manage daily referral caseloads.
  • Identify complex referrals and escalate concerns to the RN as needed.
  • Coordinate referrals to support timely patient admissions and Start of Care.
  • Communicate with hospitals, physician offices, care managers, and other referral sources.
  • Provide excellent customer service and educate referral partners on agency services and referral processes.
  • Ensure complete and accurate documentation for clinical operations, billing, and payroll.
  • Meet productivity and quality standards while supporting Catholic Health's mission and values.

Qualifications

  • Bachelor's degree in related field
  • Preferred two years of healthcare experience.
  • Experience with Microsoft Office and electronic medical records (EMR) preferred.
  • Strong organizational, communication, customer service, and multitasking skills.



Salary Range

USD $27.00 - USD $31.00 /Hr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
