About the Opportunity

We are seeking a Home Care Intake Coordinator to serve as the primary point of contact for families enrolling in the Medicaid Home Help Program.

This role is ideal for someone with home care, healthcare intake, or Medicaid experience who enjoys guiding families through the enrollment process and ensuring a smooth start to services.

You will play a key role in helping families understand eligibility, complete documentation, and move efficiently through onboarding.

Key Responsibilities

• Serve as the primary point of contact for family caregivers

• Guide families through the Home Help Program enrollment process

• Explain Medicaid program requirements, eligibility, and next steps

• Assist with completing and submitting required documentation

• Follow up with families and caregivers to prevent delays in onboarding

• Document all interactions in agency systems

• Collaborate with internal teams to ensure smooth operations
