Simpson is looking for a Home Care Coordinator to join our dynamic and seasoned team. Home care service coordinator and administrative contact to deliver the highest quality of care to our clients for 3 communities in Philadelphia, Downingtown, and West Grove. This position performs a variety of tasks under the general guidance of the Home Care Director, including but not limited to scheduling, management of client care, communicating with clients and staff, staff supervision and general office duties.

  • Scheduling
    • Create, implement, and maintain both care partner and client schedules.
    • Staff cases matched by clients' immediate needs and care plan, care partners' skills, experience, availability, and personality.
    • Manage and communicate staff schedules to ensure staffing is sufficient and meets client’s care plan
    • Manage and communicate client schedules to meet their immediate care needs and care plan.
    • Effectively communicate scheduling changes to care partner and client to ensure continuity in care and prevent disruption in service.
    • Provide caregiving services when needed to maintain schedule in accordance with client’s care plan.
    • Coordinate all assessments, new client service, and ongoing service to meet the needs of client.
  • Administrative
    • Maintain accurate documentation including records in electronic medical record and scheduling platforms, billing and payroll hours, client care, staff performance and attendance.
    • Manage and maintain both care partner and client records in accordance with corporation and all homecare licensing requirements.
    • Assist with preparing client and staffing information for Department of Health licensing renewal survey.
  • Staffing
    • Interview, onboard, orientate and train new staff members.
    • Communicate with Home Care Director regarding staffing needs to ensure client needs are met and promote growth of operation.
  • Management of Staff
    • Recognize and reward good job performance and promote care partner recognition.
    • Participate in conducting performance reviews and disciplinary plans.
    • Manage corporate and department compliance as guided by policies.
    • Address employee and client satisfaction to effectively resolve complaints and incidents.
    • Escalate disciplinary action to Home Care Director.
  • Caregiver Training
    • Orientate care partners to clients, their care plans, and their needs.
    • Assist with quarterly staff training meetings as organized by the Home Care Director.
    • Monitor status of mandated corporation and state regulated trainings for all homecare staff to ensure completion by deadlines.
  • Client Assessments and Care Planning
    • Respond to all client inquiries for services.
    • Work with internal referral sources to assess needs and establish care for new clients.
    • Create, implement, and maintain care plans and care schedules to meet the needs of client
  • Case Management
    • Communicate regularly with client, care partners, family, and interdisciplinary team regarding the client’s progress and needs.
    • Monitor client’s status and adjust care plans as needed based on client needs and level of function.
    • Participate in interdisciplinary care planning to promote the general health and welfare of clients.
  • Collaboration
    • Participate as a member of the Simpson Communities Interdisciplinary Teams and Management Teams.
    • Maintain positive relationships with coworkers, clients, vendors, and referral sources.
    • Communicate needs, deficiencies, incidents, client changes, and staffing updates to Director of Home Care.
  • Office Management
    • Promote a positive work culture that aligns with Simpson’s values of compassion, integrity, and respect.
    • Maintain an efficient, organized, and welcoming office for staff and guests.
    • Answers calls and take accurate referral/service requests and communicate with the director.
    • Maintain necessary office supplies.



Qualifications

Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Must be able to stand, sit and walk for extended periods throughout the working day. Must be able to maintain balance, kneel, crouch, grasp, bend, pull and reach for extended periods throughout the working day. Must be able to perform repetitive motions for extended periods throughout the working day. Must be able to lift, push and/or pull a minimum of 50 lbs. Requires full use of gross and fine motor skills of the upper and lower extremities.

Equipment/tools/machines used in performance of this job:

General office equipment: computers, laptops, tablets, telephone, smart cell phone, calculator, copiers, fax, communication and pager equipment and presentation equipment such as projectors.

Education or Degree Required: High School Diploma or GED

Licenses, Certifications, Registrations: Valid Driver’s License, Proof of Valid Auto Insurance and Auto Registration, a clean PA DMV driving record and Reliable personal transportation. Pennsylvania Child Abuse History Clearance



Qualifications and Related

Experience:

  • Home Care field experience (minimum 3 years)
  • Care Coordinator and/or Scheduling experience in health care field (minimum 3 years)
  • Leadership Experience (minimum 1 year)
  • Proficient in computer skills including Microsoft Suite of products
  • Be at least 21 years of age
  • Must be able to work independently while adjusting daily job priorities to satisfy the changing needs of clients
  • Travel between our Resource Center and Communities is required
