St. Thomas Community Health Center is looking to hire a non medical case manager. The Non-Medical Case Management (NMCM) provides advice and assistance to empower clients to access a variety of necessary services to support medically related outcome measurements. Clients will have varying levels of needs throughout their enrollment in services. These services may include: medical, social, community, legal and financial. Clients who demonstrate a low level of need may benefit from NMCM. Enrollment in NMCM is not always permanent; as a client may move from one type of case management to the other depending on current situations. Frequent and ongoing assessments by case management (non-medical and medical) should occur to ensure clients receive the appropriate level of care.

St. Thomas Community Health Center, a Federally Qualified Health Center (FQHC), provides comprehensive primary care for both insured and uninsured patients in the New Orleans area.
VISION. To improve the health of the underserved in New Orleans.
PATIENTS. We accept most private insurance plans, as well as Medicaid, Medicare, and GNOCHC. We also offer services on a sliding scale for uninsured and underinsured.
QUALITY. St Thomas CHC has been recognized by the National Council for Quality Assurance as a Level 3 Patient Centered-Medical Home. Level 3 is the highest level possible and is the Gold Standard in health care quality.
SERVICES. St. Thomas CHC offers a full range of preventive and primary care services. In addition to the primary care services of Internal Medicine and Pediatrics, St. Thomas CHC also offers OB/GYN, Mammography, Optometry, Cardiology, Nephrology, Rheumatology, and Mental Health Services.
