Hours: Monday - Friday 8am-4:30pm
Pick up inpatient/outpatient/emergency department encounters, and prep, scan, upload, and index the encounters. Conduct a concurrent review of the inpatient records on the nursing wings, and analyze records on the nursing wings, tag deficiencies for verbal orders and other documentation as required by Medical Staff Bylaws.
Primary Duties
1. Pick up inpatient/outpatient/emergency department encounters enter the encounters and discharges into the computer chart locator.
2. Assembles charts in the established chart order, incorporating any loose sheets received and dictation available. Verifies each page of record for correct patient name and date of admission.
3. Analyzes record on the nursing wings, tags deficiencies, and remove tags for quantitative deficiencies. Removes tags from concurrent review if complete. Enters deficiencies into deficiency tracker, prints deficiency sheets and forwards records to appropriate areas for completion.
4. Chart transcribed reports to the nursing wings. Tags the transcribed reports for signatures for completion.
5. Scans Emergency Room Department Records on a regular basis, and review encounters for misfiles.
6. Send records to storage on a periodic basis following accordance to the Health Information Services policy and procedures for retention and destruction of patient health information.
7. Scans physician queries into iDoc for request for additional information.
8. Independently recognize and performs duties which need to be done without being directly assigned.
9. Establishes priorities, organizes work and time to meet the Health Information Services workflow plan and operation.
10. Performs other duties as assigned or required.

Minimum Education/Certification:
• High School graduate
