Job Description

1. Receives all IP,OP,ER, Observation records, assembles and performs chart analysis and scanning.

1. Assures that all diagnostic studies ordered

1. Performs chart analysis identifying all deficiencies. Flag for completion, records in the computer system and prints a deficiency sheet.

4. Verifies receipt of all discharge records, assembles and performs chart analysis, processes inpatient admissions.

1. Picks up and verifies receipt of all discharge records one day post discharge from the nursing units.

1. Assembles all discharge records in pre-determined standard chart order, removing and returning all forms that are not to be a part of the permanent medical record.

1. Obtains ambulatory surgery charts from unit, assembles and performs chart analysis, processes ASC charts.

1. Picks up and verifies receipt of all ASC records 1 day post-surgery.

1. Assembles all records in standard chart order, removing and returning all forms that are not part of the permanent medical record.

1. Performs chart analysis identifying all deficiencies. Flag for completion, records in the computer system and prints a deficiency sheet

1. Completes birth certificates on all newborns born

1. Obtains information on all newborn babies in order to complete the birth certificate, and completes paternity from if appropriate, verifying information with the parents.

1. Obtains signatures from newborn parents and the physician.

1. Records all births in the birth log.

1. Assist in the retrieval of patients charts of patient studies and reviews.

1. Locates chart, signs chart out, and signs charts in upon return. Routes incomplete charts to the correct physician for completion

1. Files medical records and reports daily in the proper medical record, in the appropriate chart order. Filing completed records accurately.

1. Assists in Scanning and Verifying records scanned into system, canning all documents timely and accurately.

1. Verifies scanned images with hard copy record.

1. Appends loose documents accurately to correct scanned record.

1. When copying a medical record, ensures documents that are in color (images etc.) are copied in color.

1. Ensures confidentiality of all PHI and safeguards medical records at all times.

1. Complies with HIPAA, regulatory requirements and HIM requirements/policies/regulations at all times.
