In this specialized role, you’ll make a meaningful impact on the lives of patients living with heart failure by providing education, support, and ongoing disease management across the continuum of care. Your responsibilities will include bedside care in the hospital setting, managing new and follow-up visits in the outpatient clinic, and conducting occasional remote work from home. This position requires a strong commitment to the heart failure population, excellent clinical judgment, and the ability to work independently with a high level of self-motivation and dedication to patient outcomes.
