Job Location: Anchorage - ANCHORAGE, AK 99508JOB TITLE: Heart Failure RN
DEPARTMENT: Clinical Services
GENERAL SUMMARY OF DUTIES: A nonexempt position that provides professional nursing care for the clinic patients following established standards and practices.
SUPERVISON RECEIVED: Reports to Clinical Operations Manager of Heart Failure
SUPERVISION EXERCISED: None
TYPICAL PHYSICAL DEMANDS: Requires full range of body motion, including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing and walking for extended periods of time. Occasionally lifts and carries items weighing up to 50 pounds. Requires corrected vision and hearing to normal range with reasonable accommodations. Requires working under stressful conditions or working irregular hours. Requires exposure to communicable diseases or body fluids.
TYPICAL WORKING CONDITIONS: Frequent exposure to communicable diseases, toxic substances, ionizing radiation, medicinal preparations and other conditions common to a clinical environment.
ESSENTIAL FUNCTIONS:
1. Serve in a flexible role to support daily clinic operations and workflow.
2. Support physicians and advanced practice providers in delivering comprehensive care to patients across the healthcare continuum.
3. Manage clinic schedules, prioritizing based on patient acuity, efficiency opportunities, and optimal patient flow.
4. Serve as a clinical resource for unlicensed medical staff and other clinic personnel.
5. Support 3rd floor primary operations, including provider desktop monitoring, scheduling, and patient communications.
6. Coordinate referrals to 3rd floor providers, ensuring timely scheduling aligned with patient acuity and provider availability.
7. Perform advanced assessments to gather pertinent objective and subjective data, triage complex cases, determine acuity, and coordinate appropriate levels of care.
8. Respond to urgent or emergent clinic events in accordance with ACLS, BLS, and clinic protocols.
9. Provide patient education and connect patients with supportive tools to achieve desired clinical outcomes.
10. Under provider supervision, perform advanced interventions per facility protocols, including:
a. Initiation, titration, and discontinuation of medications.
b. Venipuncture, phlebotomy, and peripheral IV insertion.
c. Dispense sample medications in compliance with regulations and protocols.
d. Ordering labs, EKGs, and referrals.
e. Administering IV, subcutaneous, or intramuscular medications.
f. Dressing changes for VADs and central lines.
g. Other interventions that are within the scope of practice of a licensed registered nurse as directed.
11. Identify and address barriers to care, implementing supportive measures to improve outcomes.
12. Participate in heart failure focused professional development to support the organization’s mission.
13. Maintain professional licensing, certifications, and affiliations in accordance with state and national licensing boards.
14. Attend required meetings and participate in committees as assigned.
15. Maintain patient confidentiality and HIPPA compliance.
16. Perform other duties as assigned by heart failure providers and clinical manager.
PERFORMANCE REQUIREMENTS:
- Knowledge of:
- - Advanced heart failure patient populations and comorbidities.
- - Heart failure self-care principles.
- - Cardiovascular procedures, medications, and common adverse effects.
- - Regulatory requirements affecting RN scope of practice.
- - Medical equipment, safety precautions, universal precautions, and PPE use.
- Skill in:
- - Cardiovascular nursing assessment.
- - Motivational interviewing and shared decision-making.
- - Timely, evidence-based care delivery.
- - Problem-solving and action planning.
- - Aseptic/sterile techniques, phlebotomy, and IV access.
- - Medication administration and patient monitoring.
- - Accessing central lines.
- - Building collaborative relationships with patients, staff, and community partners.
- - Using electronic and paper documentation systems securely and accurately.
- - Developing and maintaining quality assurance measures.
- Ability to:
- - Maintain quality control standards.
- - Remain calm and effective during emergencies.
- - Apply clinical guidelines and protocols.
- - Communicate clearly and respectfully.
- - Prioritize tasks in a fast-paced, high-stress environment.

QualificationsEducation:
Minimum: Graduate of an accredited school of nursing.

Experience:
Minimum: One year of professional nursing experience in an intensive care or telemetry monitoring unit or clinic setting.
Certificate/License:
- Possession of a current Alaska State registered nurse license, CPR and ACLS certifications.
- Encouraged to obtain CHFN certification through the American Association of Heart Failure Nurses (AAHFN) within 2 years of gaining employment as a HF nurse.
