The Franciscan Healthy Living Center Intake Coordinator is responsible for assisting in the enrollment process for new Franciscan Health Healthy Living Center program participants. This position will support all programs and departments working with and within the Healthy Living Center.

WHAT YOU CAN EXPECT
Schedule: Monday through Friday from 8:00 a.m. to 4:30 p.m.
Describes the Healthy Living Center program, either by telephone, electronically, or in-person to interested persons including candidates for enrollment or others in the community interested in the Healthy Living Center program.
Assists patients, families, physicians and other referral sources via telephone with clinical issues, problems and questions. Document phone calls in Epic and route to clinical staff; identifies and escalates patients concerns to appropriate clinical staff as needed.
Communicates with all referral sources to gain the necessary information to facilitate continuity of care and appropriate care of patients and clients including demographics, diagnoses, physicians' orders, and reimbursement or insurance information.
Completes appropriate referral forms and enters information into the computer. Monitors referral work queues for the Healthy Living Center departments. Monitors incoming referrals by fax and tracks following department guidelines.
Coordinates patient care and navigates patients through various departments within the healthy living center and hospital. Performs a variety of administrative functions to support the department, including communication of problems and backlogs to the manager or director as needed.
Makes arrangements and schedules meetings, conferences, conference calls and appointments. Assists with preparation and sending notices, agendas and other prepared materials to appropriate persons. Receives and documents RSVPs.
Answers multi-line telephones answering the callers' questions or directing the caller to the appropriate party.
Files documents in accordance with established filing systems in a timely manner.
Develops and oversees inter-and intra-departmental communications, special projects, and services. Sends patient letters and other communications per clinic processes or as directed. Drafts memos, emails and marketing materials to meet department needs. This includes, but is not limited to, newsletters and support group documents. Communicates problems and concerns to the manager/designee.
Enters information into various systems; runs and distributes reports as appropriate. Compile quarterly data for various programs.
Manages inventories and orders supplies. Oversees maintenance and quality checks of the department's equipment and supplies. Communicates problems and backlogs to manager or director as needed.
Assists the department with special events such as open houses, health fairs, or outreach needs.
Assists with coordination and creation of newsletters or patient documents as directed by department manager. Create drafts, revise and edit marketing materials and documents for new programs as directed in collaboration with leadership.
Acts as a resource for processing registration information, insurance verifications, billing claims and issue, and documentation guidelines, including advising or referring providers to appropriate sources.
