Job Description

1) SUMMARY OF DUTIES

Assists the qualified nurses in providing care to all patients within an assigned unit of the hospital and such nursing care is an integral part of the individualized holistic patient care plan as directed by the Medical staff according to the Policies and Procedures of the hospital and in keeping with the generally accepted principles, policies and procedures of the Nursing Services.

2) JOB DESCRIPTION

Organization & Management

  • Understands and carries out the nursing care in line with Vision and Mission Statements, Philosophy, Goals and Objectives that suit the scope of the Nursing Services and hospital.

  • Understands the service organization structure.

  • Informs Nurse-In-Charge or Unit Manager when changes occur in circumstances which affect standards of care.

  • Receives ward report and assignment before the change of each shift.

  • Attends meetings held by the services in order to discuss issues and matters pertaining to the operation of the service.

  • Daily routine:

- Performs regular nursing rounds on all patients in the wards to ascertain for other additional needs, talks to patient/family members.

- Makes and tidies patients’ beds. Change linen when necessary.

- Carbolizes beds and equipment and tidies patient’s bedside lockers, cardiac tables, chairs and surrounding areas.

- Maintains the cleanliness of the ward, pantry, treatment room, utility and linen room.

- Collects, cleans and fills hot water flasks for patients.

- Dispatching: charge tickets, CSSS items, specimens, etc. to relevant services.

  • Answers phone calls and relay messages.

  • Responds to patient’s call bell promptly.

  • Assists the qualified staff in the following:

- Carries out basic nursing care for patient’s comfort, safety, nutritional and elimination needs.

- Admission, transfer and discharge of patients.

- Gives bed bath and provide total personal hygiene to non-ambulatory patients.

- Assist ambulatory patients when necessary.

- While doing dressing, setting intravenous lines, catheterization, vulva swabbing etc.

- Clears the trolley after use.

- Washes instruments used and place in the appropriate place.

- Recording daily charges for disposable/ equipment / consumable items used in the ward. into patient’s charges accordingly.

- Maintaining record of intake and output chart.

- Lifting, turning and positioning of patients in bed, bed to chair and bed to trolley using the correct body mechanics.

- Treatment of pressure sores.

- Taking diet orders for new admission and serving meals and drinks, collects trays.

- Helps to feed patients orally.

- Prepares drink for patients at night and when necessary.

- Chaperons Consultant in doing procedure and ward rounds in the absence of trained nurses.

  • Accompanies the nurses to send patient to OT and from OT.

  • Escorts patients to other services for diagnostic investigations.

Human Resource & Development

  • Takes personal responsibility for own professional development and keep up to date with professional change and growth by maintaining mandatory learning hours.

  • Participates in the orientation of new members of staff to the ward routines.

  • Participates actively in any other new service that is introduced.

  • Maintain the record of personal leave and training attended.

Policies & Procedures

  • Adheres to the hospital policies and procedure and nursing code of conduct.

  • Maintain confidentiality of patient’s personal and medical history.

  • Ensure adhere with Prevention and Control of infection.

Facilities & Equipment

  • Assists the qualified staff in the following:

- Maintains proper storage of biomedical equipment assigned in the unit.

- Check all equipment to ensure that they are in good condition and ready to be used at all time. Report if there is any faulty.

- Maintains adequate supply of ward stock.

- Keeps accurate records of equipment, medical supplies and drug in the Unit/ wards.

Safety & Quality Improvement Activities

  • Complies and promotes Baby Friendly Hospital Initiative Breastfeeding Policy, and protect, promote and support Baby Friendly Hospital activities.

  • Participates actively in any other new service that is introduced by the services.

  • Participates in 5S activities and comply with the specified requirements Practices Standard People Practice (SPP) at all times.

  • Ensure compliance of Patient safety goals as safety improvement measures recommended by World Alliance for Patient Safety.

  • Policies on patient and family rights are addressed at all times.

  • Participates actively and encourages compliance to all policies and Procedures of ISO9001: 2008, ISO 14001, OHSAS 18001, ISO 14001:2004, Malaysian Hospital Accreditation Standards, Joint Commission International Accreditation Standards, PSG and NNA.

  • Complies with any instruction or measure on occupational safety and health.

  • Carries out duties as an employee as stipulated in Occupational Safety and Health Act and Regulations 1994:-

- To take reasonable care at work for the safety of yourself and other persons; - To co-operate with your employer or any other person in the discharge of any duty;

- To wear or use any protective equipment or clothing provided by the employer;

- To comply with any instruction or measure on occupational safety and health.

  • Ensure all policies and procedures of ISO 37001 Anti Bribery Management System (ABMS) are adhered to and responsible for integrity principles, practices, and activities at hospitals & companies if and when these programs are implemented.

3) JOB REQUIREMENT

  • Possess academic qualification of SPM qualification.

  • Certificate in Care Assistant.

  • Able to converse fluently and write in English language and Bahasa Malaysia.

  • Experience working in hospital setting is an advantage.

  • Have interpersonal and communication skills.

  • Caring, honest and responsible worker.

  • Experience working in hospital is an advantage.

  • Willingness to work on scheduled hours, shift, and weekends, public holidays and anytime when called during emergency.

  • Has good communication, counseling and interpersonal skills.

  • Able to guide new Care Assistant, student nurse and other related support staff.

  • Possess the attitude to make the patient care the primary concern, treating every patient compassionately, demonstrating and an aspiration to altruism.

  • Seek to treat, maintain patient’s dignity, privacy, autonomy and confidentiality.

  • Prepared to work as a team member.
