Job Description

1) GENERAL DESCRIPTION

Responsible for rendering professional nursing care to patients within an assigned unitofthehospital.Suchnursingcareisanintegralpartoftheindividualizedholistic patient care plan as directed by the medical staff according to the Policies and Procedures of the hospital and in keeping with the generally accepted principles,policiesand procedures of the Nursing Services.

2) JOB DESCRIPTION

Organization and Management

  • Understands and carries out the nursing care in line with Vision and Mission Statements,GoalsandObjectivesthatsuitthescopeoftheNursingServices.

  • WorksshiftdutiesorasassignedbyUnitManageraccordingtoUnit’sOperational Hours.

  • InformsUnitManagerwhenchangeoccursincircumstanceswhichaffect standards of care.

  • Receivesandhandoverreportofpatient,knowsthecaseindetail:

- Progressofthepatient.

- Ensures all doctors’ orders are carried out before shift change and ontaking over case.

- Anyincidentorproblemsencountered.

  • Visitseverypatientintheunittogetherwiththeteammembersontakingover of duties to:

- Assessconditionofpatient,comparewithreport.

- Re-plannursingcareaccordingtoassessmentorevaluation.

- Determinesnursingprioritiesandplanspatientcareaccordingtopatient’s needs and staff availability.

  • Provides individualized holistic care to patients using the Nursing Process approach with proper nursing documentation in patients’ files and that it iscontemporaneous.

  • Reportsallabnormalfindingsandprogressofpatientstotherelevantdoctors or consultants.

  • Conducts regular nursing rounds on all patients in the ward to ascertain for other additional needs; talk to patient/family members as an integral part of providing emotional support.

  • Follows doctors / consultants ward round and assists them in doing procedures when required.

  • Carries out orders of doctors / consultants pertaining to care of patients and delegates to other team members according to their assignments.

  • Establishesanddevelopseffectivecommunicationwithalldisciplines,patients and relatives, to achieve optimal patient’ satisfaction.

  • Attends and participates in unit meetings, dialogue sessions and hospital’sactivities.

  • Participatesinthehealtheducationofpatientsandtheirfamiliesintheward.

  • AssiststheUnitManagerintheadministrationoftheunitwardswhen assigned as shift nurse in charge.

  • Acts on behalf of the Unit Manager in setting standards and expectations in the quality of care.

  • Carries out duties and responsibilities of the Unit Manager in her absence asdirected.

  • Ensurechargesofequipment/consumableitemsintopatient’scharges accordingly and checked for completeness.

  • ResponsibleforcheckinganddocumentingsupplyofsterilepacksfromCSSS and weekly checking for integrity of packaging.

  • Maintainsanddocumentsadequatesupplyofunitstock.

  • CarriesoutassignmentsscheduledbyUnitManagerandprovidefeedback.

  • Liaisesanddiscusseswiththerelevantdoctorsconsultantswithregardsto care of patients in the unit / ward.

Human Resource and Development

  • Takespersonalresponsibilityforownprofessionaldevelopmentandkeepsup to date with professional change and growth by maintaining mandatory learning hours.

  • Participates in the teaching program of the unit / ward and orientation of new members of staff to the ward routine.

  • Involvesinthesupervisionofthelowercategoriesofstaff such as Assistantnurses,Midwives,CareAssistant, Studentnurseswhoarepostedtotheunitfortheirclinicalexperience andassistthemtoachievetheirobjectivesofthatclinicalattachment.

  • Assists the Unit Manager in appraising the job performance of the lower categories of unit staff by giving accurate feedback with no malice orprejudice.

  • Participatesinthedevelopmentandmaintenanceofthenursingprocess when providing patient care as well as nursing documentation.

  • Supports and assists the Unit Manager in effective and efficient utilization of staff, supplies and services

Policies and Procedure

  • ContributessuggestionsinperiodicrevisionandupgradingofNursingPolicies and Procedures and relevant work instructions.

  • Checks DDA drugs with the off-going Nurse In-charge of the previous shiftwhen in-charge of the present shift.

  • Participatesinthecare,custody,andadministrationofmedicationsin accordance with Hospital Policies and Procedures.

  • Checksdailyandauditsresuscitationtrolleyperiodicallyasscheduled.

  • Ensure all staff complies with Prevention and Control of Infection includingimplementationofsurveillanceactivityintheunitandevidenceofinvolvement.

  • Maintainconfidentialityofpatient’spersonalandmedicalhistory.

Facilities and Equipment

  • Assistthequalifiedstaffinthefollowing:

- Oversees the storage and condition of biomedical equipment within the unit,inspectingitregularlytoensureitmeetsthePPMschedule,reports any faulty equipment, and is ready for use.

- Overseeing the linen and medical supply store by ensuring adequate stock and linen supply according to par level, monitoring stock movement and tallying after charging, coordinating monthly internal stockcounts,andmaintainingaccuraterecordsofequipment,medical supplies, consumable items, and laboratory items for specimencollections.

  • Participateindoingastockcountwiththefinanceteamquarterly.

Safety and Quality Improvement Activities

  • Operatesmaintainsspecializedequipmentassignedtotheunitandprovides assistance guidance to new staff in their use.

  • Assists the Unit Manager in keeping accurate records of equipment, medicalsuppliesand drugs in the unit / ward.

  • Checks that all equipment in the unit are functioning well and service according to PPM schedule.

  • Report to Bio Medical personnel if any faulty and takes necessary actionaccordingly.

  • Assists in maintenance of instruments and equipment by proper cleaning, handling, disinfect and utilizing as directed.

  • Fully receptive and supportive in the activities that contribute towards continuous improvementofnursingservicee.g.,QualityAssurance,Infection Control and Fire and Safety Program.

  • Conduct investigation androotcauseanalysisinanyincidentintheservices, takes appropriate action and ensure completion of incident reports within stipulated time frame.

  • Carries out duties as an employee as stipulated in Occupational Safety and Health Act and Regulations 1994: -

- Totakereasonablecareatworkforthesafetyofyourselfandotherpersons.

- To co-operate with your employer or any other person in the discharge of any duty.

- Towearoruseanyprotectiveequipmentorclothingprovidedbytheemployer.

- Tocomplywithanyinstructionormeasureonoccupationalsafetyandhealth.

  • EnsurescomplianceofPatientSafetyGoalsassafetyimprovementmeasures recommended by World Alliance for Patient Safety.

  • EnsuresthepoliciesonPatientandFamily’sRightsarealwaysaddressed.

  • Complies with the Baby Friendly Hospital Initiative Breastfeeding Policy, and protect, promote, and support Baby Friendly Hospital activities.

  • Participatesin5Sactivitiesandcomplywiththespecifiedrequirements.

  • EnsuresallstaffalwaysadherestoStandardPeoplePractice(SPP).

3) JOB REQUIREMENT

  • RegisteredwithNursingBoardofMalaysiawithavalidlicensetopractice as a Registered Nurse.

  • PossesscertificationsinBLS,ACLS,andPALS.

  • ProficientinwritingandconversinginEnglishandBahasaMalaysia.

  • Microsoftofficeliteracy.

  • Haveinterpersonalandcommunicationskills.

  • Criticalthinkingandproblem-solvingskills.

  • Caring,honest,andresponsibleworkers.
