The purpose of this position is to manage and oversee the daily operations, health services, and clinical support functions of the Adult Day Care program while providing health and medical support to Senior Center participants as needed. This position is responsible for supervising assigned staff, coordinating participant health services, overseeing health assessments and care needs, and promoting the health, safety, and well-being of participants.

The position ensures services are provided in accordance with applicable regulatory and licensing requirements, departmental policies and procedures, and established standards of care.

This class works independently, under limited supervision, reporting major activities through periodic meetings.

Examples of duties

The essential functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the class as necessary.

  • Manages and oversees the daily operations and health services of the Adult Day Care program.
  • Supervises, directs, schedules, and evaluates assigned staff; provides guidance, counseling, and corrective action as necessary.
  • Conducts and oversees participant health assessments, routine health monitoring, nursing services, and other health-related needs within the appropriate scope of practice.
  • Provides health and medical support to Senior Center participants and staff as needed, including responding to health concerns and emergencies.
  • Coordinates participant health services with caregivers, physicians, healthcare providers, case management staff, and community resources.
  • Oversees participant admissions, care plans, health documentation, and ongoing reviews to ensure participant needs are appropriately addressed.
  • Ensures compliance with applicable licensing, regulatory, departmental, and program requirements, including appropriate staffing and documentation.
  • Participates in regulatory monitoring visits and inspections, provides required supporting documentation, and assists with corrective actions as needed.
  • Plans and coordinates health education, screenings, wellness programs, and other health-related activities for Adult Day Care and Senior Center participants.
  • Provides staff training and guidance related to participant health and safety, infection control, emergency response, and health-related procedures.
  • Monitors and addresses participant health and safety concerns, incidents, and changes in condition and ensures appropriate documentation and follow-up.
  • Participates in the development and implementation of policies, procedures, quality improvement initiatives, and emergency preparedness activities.
  • Attends required departmental and interdisciplinary meetings, continuing education, conferences, and professional development activities.
  • Identifies and coordinates resources and partnerships that support Adult Day Care, health, and wellness programs.
  • Other duties, responsibilities, and activities may change or be assigned at any time with or without notice.

Qualifications

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Bachelor’s Degree in Nursing, Human Services, Social Services, or related disciplines.
  • Four (4) years of experience in nursing, human services, social services, or other health environments or an equivalent combination of education, training and experience in a related setting.
  • Minimum of two (2) years of supervisory or managerial experience in related work setting.
  • Must possess and maintain a valid state driver’s license with an acceptable driving history.

PREFERRED EUDATION AND EXPERIENCE

  • State of Florida Registered Nurse or Licensed Practical Nurse (RN or LPN) License.

Supplemental information

KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of nursing techniques and practices in dealing with older people and in completing health assessments for seniors.
  • Knowledge of community resources and medical/health providers in the community.
  • Skill in administering medication or supervising this task.
  • Ability to supervise nursing staff, provide health education, counseling and screening for seniors.
  • Ability to plan, organize and coordinate health services with medical providers

PHYSICAL DEMANDS:

Physical demands refer to the requirements for physical exertion and coordination of limb and body movement.

  • Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

WORK ENVIRONMENT:

Unavoidable hazards refer to the job conditions that may lead to injury or health hazards even though precautions have been taken.

WORK HOURS

Monday – Friday 8am- 5pm (May be required to work nights, weekends and holidays to meet the business needs of the City.)
