Qualifications
QUALIFICATIONS:
 
Education Required: Bachelor’s Degree in human services / related discipline required. Master’s Degree in preferred
 
License(s) Required None
 
Experience/Skills
  • A minimum of two (2) years of experience in case management is required.
  • Prior experience in the Health Home model preferred.
  • Excellent communication skills, verbal and written
  • Working knowledge of physical and/or mental health
  • Ability to travel to meet with patients
  • Proficiency in electronic data collection systems
  • Bilingual Spanish/English preferred
