Job Description

JOB SUMMARY

The Population Health Clinical Assistant provides administrative and care coordination support to PHM nurses, the clinical pharmacist, social worker, and nurse case managers. This role organizes referrals, schedules member assessments, coordinates communications, and tracks follow-up tasks so clinical staff can focus on member care.

KEY RESPONSIBILITIES

  • Receive, enter, and route referrals to the appropriate PHM team member.
  • Contact members to schedule assessments, appointments, and follow-up calls with clinical staff.
  • Organize work queues, track assigned tasks, and notify clinical staff of outstanding needs.
  • Request medical records, discharge summaries, and other information from providers and facilities.
  • Assist with coordinating provider appointments, transportation, and community resources as directed.
  • Schedule interdisciplinary care meetings and distribute invitations and care plans as directed.
  • Support pharmacy and social work referrals by gathering information and tracking follow-up.
  • Prepare and send member and provider correspondence and educational materials.
  • Maintain accurate documentation and assist with reports, outreach tracking, and audit preparation.
  • Refer clinical questions and urgent concerns promptly to appropriate licensed staff.
  • Perform other job-related duties as required or assigned.

QUALIFICATIONS

  • Strong organizational, customer service, communication, and teamwork skills.
  • Ability to manage multiple priorities and communicate respectfully with members and caregivers.
  • Knowledge of medical terminology and basic healthcare processes.
  • Proficiency with Microsoft Office and electronic documentation systems.
  • Ability to maintain confidentiality and perform nonclinical duties under clinical staff direction.
  • Successful completion of Health Care Sanctions background check.

EDUCATION/EXPERIENCE:

  • High school diploma or equivalent required.
  • Educational background in healthcare or a related field preferred.
  • One year of experience in healthcare administration, member services, medical office support, or a related setting.
  • Experience supporting care management, referrals, scheduling, or health plan operations preferred.
