Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of Staten Islanders by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – the poor and working poor, low-income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, and the LGBTQ community.
Community Health Action of Staten Island, in partnership with Sun River Health, has developed a collaborative program to enhance care engagement, improve HIV health outcomes, and promote the well-being of people living with HIV/AIDS (PLWH/A) in Queens and Staten Island. The Ryan White Support Services program addresses unmet social service needs that have significantly hindered their access to and engagement in HIV and other essential medical care. The Health Educator with the support of the Comprehensive Services Coordinator, Community Navigators, and Peer Advocates, will collaborate with the medical and case management teams at the Sun River’s Health Center on Staten Island and Sutphin Health Center in Jamaica, Queens. This program will expand services to Staten Island and establish community-based, non-Medicaid eligible services in Queens to assess, identify, and address unmet social determinants of health (SDOH) needs. The Health Educator will be responsible for facilitating NYS Department of Health-approved HIV Health Education/Risk Reduction curriculum in small groups, food and nutrition counseling/education, and developing an individualized services plan.

Community Health Action of Staten Island drives dramatic improvements in the health of Staten Islanders.
In 1988, a group comprised of Staten Islanders who were concerned about the impact of AIDS on our community came together to address the stigma, the discrimination, and terrible conditions of people with AIDS on Staten Island. This group of activists, providers, family members, and people living with the virus to mount created the Staten Island AIDS Task Force.
In 2004, we changed our name from the Staten Island AIDS Task Force to Community Health Action of Staten Island to reflect the expansion of our services as we respond to the evolving needs of the people we serve.
Over the years, our services expanded. We now offer a wide range of services, including domestic violence counseling and intervention, a 24-hour resource and recovery center, alternatives to incarceration services, initiatives for the LGBTQ community and youth, peer education and syringe exchange, opioid overdose prevention trainings, HIV prevention education, benefits assistance for SNAP and health insurance enrollment, community-based HIV, hepatitis C and STD testing, a food pantry, and a mobile food pantry.
The agency operates in neighborhoods with the highest rates of HIV, poverty and substance use. We also operate programs on Mobile Health Units, in NYS Correctional Facilities, at the Pride Center of Staten Island, and the Staten Island Family Justice Center.
