THIS IS A GRANT-FUNDED POSITION FUNDED UNTIL DECEMBER 31, 2026.
ESSENTIAL JOB FUNCTIONS: The Health Coach assists with health screening and health coaching using motivational interviewing techniques to support participants at the Health Hubs in the neighborhood Health Hub program. This position collaborates with clinicians and participants to design individual programs as well as make referrals to necessary primary and behavioral health care and other community supports.
Establishes trusting relationships with participants to encourage engagement with individualized programs and healthcare providers for improved health outcomes.
Assesses and addresses barriers to access preventive care.
Collaborates with medical professionals and program leads to create educational materials for participants.
Educates/coaches program participants on disease prevention and advocacy for improved health care.
Coordinates services with internal and external behavioral and mental healthcare providers.
Conducts virtual and/or in -person follow-ups to promote continued engagement and retention.
Generates referrals to community supports and works with community organizations and rental offices to recruit program participants.
Collects and enters data into the database.
Assists with the front desk as needed.
Performs other duties as assigned.
EDUCATION: High School Diploma or GED. (Associate’s or Bachelor’s Degree in Health, Social Services, Human Services, or a related field is preferred.) (TRANSCRIPT REQUIRED)
EXPERIENCE: Two (2) years of related work experience. Must have a valid Driver’s License. (Experience as a Community Health Worker or related job specializing in chronic disease management or prevention related to obesity is preferred.)
KNOWLEDGE, SKILLS, AND ABILITIES:
WORK SCHEDULE: This position may frequently be required to work weekends and evenings. May require occasional overnight travel.
