About the Role

As a Health Coach, you will engage and support patients in adopting sustainable, healthy lifestyle changes. This role involves providing consistent accountability and guidance through phone, text, and in-person interactions. The Health Coach will assist patients in implementing and maintaining complex lifestyle recommendations, focusing on key health pillars: sleep, nutrition, mind, growth, movement and environment. The ideal candidate is a people-oriented individual who thrives on building relationships, is deeply committed to improving health outcomes, and brings a strong foundation in coaching, healthcare, or personal wellness. They should be teachable, approachable, and confident in understanding human psychology and motivation.

Responsibilities

• Regularly engage with patients via phone, text, and in-person meetings to provide accountability and encouragement.

• Follow the doctors treatment plan to guide patients in implementing personalized, in-depth lifestyle recommendations designed to improve health and well-being.

• Help to answer and questions, translate and clarify recommendations given by the doctor.

• Build trusting, transparent relationships with patients that allows them to feel safe and cared for, so that they are motivated to follow guidance.

• Educate patients on health pillars, including sleep, nutrition, movement, mental and emotional health, and relationship building.

• Motivate and inspire patients using proven coaching techniques to help them overcome challenges and stay on track.

• Listen actively to patients concerns and respond with empathy and diplomacy, and provide them with the doctors actionable advice.

• Collaborate closely with the team to align coaching strategies with clinical goals.

• Adapt to and learn from feedback, remaining open to constructive criticism and continuous professional development.

• Foster a sense of community and care, building trust and rapport with each patient.

Qualifications

• Relevant training or certification in health coaching, lifestyle coaching, motivational coaching, food science, nursing (RN, LPN), or equivalent experience in health and wellness.

• Strong foundational knowledge of health and wellness, with practical experience implementing and coaching lifestyle changes.

• Demonstrated understanding of human psychology, motivation, and behavior change techniques.

• Proven ability to navigate challenging interactions with diplomacy and professionalism.

• Exceptional listening, communication, and interpersonal skills.

• Energetic and outgoing personality with a natural ability to connect with people.

• Deep empathy and understanding of the challenges individuals face when unwell.

• Team-oriented, with the ability to collaborate and thrive in a multidisciplinary environment.

• Growth mindset, open to learning, and adaptable to new strategies and feedback.

• Previous experience in a coaching or mentorship role is preferred.

• Passion for health and wellness, with personal experience in adopting and maintaining a healthy lifestyle is a plus.
