Key Responsibilities

Day-to-day responsibilities of this position:

Are you a Registered Nurse with experience working with children and youth? Do you want to collaborate with various stakeholders to ensure Utah's youth are receiving the care they need? The Department of Health and Human Services is seeking to hire a passionate individual to fill their role of Health Care Coordinator.

  • Assessing and evaluating a patient's clinical problems and health care needs
  • Providing care oriented toward improving or maintaining health and preventing illness
  • Providing clients with information concerning rights, options, benefits, services, goals and expectations
  • Maintaining and updating patient charts, records, and information
  • Providing consultation and/or expert advice or testimony
  • Collaborating with various stakeholders, such as DCFS, Medicaid, and medical professionals to ensure the client's
    healthcare needs are met

Schedule:

  • Monday - Friday from 8:00AM - 5:00PM
  • This position is currently a hybrid of both in-office and remote work days. Please note, a position's eligibility for
    remote work is established by agency management and is subject to change at their discretion at any time and for
    any reason.

Qualifications

Minimum or "day-one" qualifications:

  • Must be a Registered Nurse
  • At least 2 years of experience working in a healthcare related program.
  • Experience working with children and youth.

Preferred or trainable/on-the-job qualifications:

  • Experience assessing a patient's clinical problems and healthcare needs
  • Experience maintaining and updating patient charts, records, and information
  • Experience providing clients with information regarding options, benefits, services, goals, and expectations.
