The Health Assistance Program (HAP) Care Coordinator is responsible for facilitating access to health insurance, medications and other health care needs for all health assistance program patients within the UVM Health Network. This responsibility will also focus on those patients who are uninsured or underinsured. This work entails individual patient care coordination as appropriate and collaboration with other health care professionals and pharmacy team members. This coordinator will ensure patients have access to resources, services, affordable medication and equipment to increase their health, independence and reduce the need for inpatient hospitalization and Emergency Department use. Serves as an important link between patient, healthcare teams, payers and the community.
EDUCATION:
High School/GED plus one year of post high school education preferred or related work experience considered
PTCB/NHA Certification encouraged/preferred
Licenses: Vermont Board of Pharmacy Technician in good standing required
EXPERIENCE:
Minimum of three to five years experience in health care, case management, pharmacy or patient advocacy
Insurance and benefit enrollment experience or direct social work experience desired
Care coordination and customer experience preferred
This is a bargaining union position.
