The Healthy Outcomes Team is a health advocacy team looking fordedicatedfriendly colleaguesto helptheMedicaidpopulation learn about and complete important health screeningsand activities

As aHealth Advocateon the Healthy Outcomes Team, you will interactwith memberstelephonicallyto inspire healthy behaviors Ourgoaloneachcallis togive everymember a clear understanding oftheirbenefits for specific screenings,medications, activities, and follow up care.

You will talk toprovidersand memberstoassistwithimportant health serviceslikesettingappointments, fillingmedications,andprovidingresources to improve Star health outcomes.

Your key responsibilities are:

  • Inbound and outbound calls to membersand providers to helpschedulingserviceslikehealthcare visits,screenings, vaccinations, or medication refills

  • Working through barriers that stand in the way of our members’ good health

  • Delivering the best member experience through professional empathetic conversations

  • Providingimportant education to members to help them make good health decisions that improve their health

  • Accurately documentingconversations in the member’s health record

Your key abilities are:

Required Qualifications:

  • Professionalcommunication skills

  • Strongactivelistening and interpersonal skills

  • High computer literacy, including ability to troubleshoot issues

  • Proficiencywith Windows and web-based applications and digital communication tools

  • Reliable high-speed internet connection (at least 50mbps) and suitable remote workspace

  • Hard wired connectivity to computer isrequired(DSL, satellite, and wireless broadband are notpermitted)

Preferred Qualifications:

  • Bilingual or multilingual candidatesheavilypreferred.

  • Experiencewith HEDIS screening measures

  • Experience withbehavior change

  • Experience working in mental health background

  • Experience working in community health

  • 1+ year experience with production expectationsor high-volume calls heavily preferred

  • 2+ yearsexperiencewithcall centeror outreach involving members/consumers

  • 3+yearsexperiencein ahealthcaresetting

  • Medical certification (certified nursing assistant, medical assistant, etc.)

Education and Certification Requirements

  • High School Diploma or G.E.D.

REQ Notes:

All applicants must have adequate internet speeds and willbe requiredto HARD WIRE their computer into their modem for this position.

Training is expected to last 4 weeks, is virtual, andmaybe ta different shift than the candidate is hired to work (this change would be temporary until training is complete).

Applicants must reside in KY, LA, VA, and IL and have the ability to work one of the following shifts

  • 9a-5:30p

  • 10a-6:30p

  • 11a-7:30p

  • 12p-8:30p

  • 1p-9:30p

Pay Range

The typical pay range for this role is:

$17.00 - $28.46

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
